How We Collect Personal Information

The Manor Clinic uses Timely Ltd (Appointment Booking Software). You will be required to verify your contact number via automated SMS containing an access code to schedule an appointment. The Manor Clinic requires you to provide personal information including your full name, email address, requested services, selected appointment time and agree to both our Privacy Policy and Cancellation Policy to successfully schedule an appointment.

An initial deposit is required to secure an appointment via a secured payment page. You will be required to provide payment details. Payment data is encrypted and Timely Ltd follows strict Payment Card Industry guidelines to keep your card information safe and secure. Prior to your scheduled appointment, you will be provided with a Patient Details Medical History & Consultation Intake Form. This is required to be completed prior to in-clinic services. This may be completed electronically before you arrive or electronically/physically in the clinic.

The Manor Clinic may collect information during the course of services and during any communication pre and post services.

 

Usage & Storage Of Personal Information

The main reason we use collected personal information is to deliver services to you. We may also use your personal information for related business activities, including, but is not limited to, audits, accreditation, internal staff training or advertising, where appropriate.

The Manor Clinic may need to disclose your personal information when required and authorised by law, to third parties who work with us to provide medical services, to other health care providers, when it is necessary to protect your health and safety or the health and safety of the public.

 

Access To Your Personal Information

If you request a copy of your medical file we require this request in writing. We will acknowledge your request within 7 days. The cost associated with providing this information may be passed on to the requesting patient. This cost covers the time spent and copying expenses. Information can be expected to be provided within 14 days from the acknowledgement of the request.

 

Storage & Protection

The Manor Clinic will take all reasonable measures to keep your personal information secured. We use cloud-based software that has implemented multiple layers of protection. Infrastructure includes servers that are protected by state-of-the-art physical security, network and edge protection by world-class providers and automated security monitoring and alerting. Compliance meets Australian Security Standard. However, no data transmission over the Internet or wireless network can be guaranteed.

Therefore, while we strive to protect your Personal Information, you acknowledge that (i) there are security and privacy limitations of the Internet which are beyond our control; (ii) the security, integrity, and privacy of any and all information and data exchanged between you and the Website and Services cannot be guaranteed; and (iii) any such information and data may be viewed or tampered with in transit by a third party, despite best efforts.

Physical copies of personal information will be destroyed if no longer required by law to maintain. All staff and contractors follow strict internal policies as part of their contract.
